This registration card contained 20 questions on the front and required the following information:
- Name
- Address
- Age
- Date of birth
- Race
- Occupation
- Citizenship status
- Nearest living relative (name and address)
- Name and address of employer
- Physical appearance (height, build, hair and eye color, physical disabilities)
